What it actually means
A commercial product taken as a starting point and modified for the application. The label deserves suspicion: modification voids much of what made the off-the-shelf item attractive — its qualification heritage, its vendor support, its production learning curve — while the "it's mostly COTS" framing hides that from the schedule.

Make/buy trades and proposals, usually near optimistic cost estimates.
